Emmanuel House’s ‘Mental Health in the County‘ team provides outreach mental health support to people who are homeless in South Nottinghamshire

John*, who is in his early 20s, had been living with his parents. His mental health was deteriorating and he was suffering with psychosis. He was unable to continue living with them due to behaviour linked to his mental health and learning disabilities. As a result of this he moved into emergency hostel provision. He was referred by the hostel to our worker at Emmanuel House for additional mental health support.

When Asia, our Mental Health in the County Support Worker, met John, she connected him with a local GP and specific mental health services. He is now working with the Early Intervention in Psychosis (EIP), a multi-disciplinary team set up to seek, identify and reduce treatment delays at the onset of psychosis.

Asia ensured that the Mental Health Crisis Team visited him every morning at his hostel to monitor and assess his ongoing mental health difficulties. As a result of Asia’s interventions and coordinating services, John now has a Community Psychiatric Nurse, a Community Worker and a Social Worker.

John’s mental health is now more stable and is still maintaining his accommodation in the hostel.
